• Fixed a bug that caused some enemies with a melee attack to create a volley of fire and frost missiles while on Insanity difficulty while having a certain empowerment. – which would instantly kill you

• Fixed a bug that stopped doorways from opening again after using them if you go back to a previous level and back again while the doors are closed- this could get you permanently stuck.
